We have also made available a completed copy of the Sunderland LEA form OS6-Educational Use Of External Outdoor Activities Providers. This is required by many e.v.c.'s prior to a visit. The form is in word format to enable you to change your name etc. Your EVC may require a different version of the form depending on your LEA.
